Thomas Splint is a reliable and widely used orthopedic device designed for effective immobilization and traction of femoral (thigh bone) fractures. Although highly effective, prolonged immobilization with a Thomas splint may sometimes lead to skin ulceration in the area under and around the splint. Studies indicate that appropriate pressure care significantly reduces the risk of skin changes. Evidence suggests that the interval for pressure care may be safely adjusted from every 2 hours to approximately 4–6 hours, potentially minimizing unnecessary movement of the fractured limb while maintaining skin integrity. Factors such as patient weight, leg circumference, fracture type, traction ring size, and traction weight can also influence the occurrence of skin complications.
